Advisa Care Hospice and Palliative Care Program seeks a licensed Nurse Practitioner in Michigan to join our team. The NP will perform home visits, conduct evaluations, order and interpret tests, prescribe within scope, and coordinate care with the Interdisciplinary Team.
Responsibilities include patient goal setting, care planning, documentation within 24 hours of visits, and collaboration with the Medical Director.
