Our focus is delivering optimal, individualized rehabilitation by highly skilled professional therapists while developing long‑term partnerships with the communities they serve.

We are seeking a Full Time Physical Therapist to join our team at Grand Village Care Center in Grand Rapids, MN (also considering full‑time, part‑time, PRN).

• Provide screenings and evaluations to determine requirements for skilled physical therapy intervention relative to the patient’s prior level of function, covering gait and balance, mobility, range of motion, strength, pain, home assessments, discharge planning, maintenance program development, and other related patient care issues.
• Develop and implement treatment plans that focus on functional and measurable outcomes, adhering to professional standards of practice.
• Monitor patients’ progress and adjust treatment plans as needed.
• Provide appropriate supervision for physical therapist assistants in compliance with licensure, regulatory, and company requirements.
• Maintain accurate and up‑to‑date patient records.
• Collaborate with, coordinate, and participate in training programs for community staff, patients, families, and rehabilitation staff.
• Participate in continuous quality improvement activities.

• Successful completion of an accredited Physical Therapy program.

• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Strong problem‑solving and critical‑thinking skills.
• Compassionate and patient‑centered approach to care.

• Current valid Physical Therapist license to practice in the appropriate state.
