• Travel daily to assigned MN service areas to visit clients in their private homes.
• Assess communication and swallowing disorders, then develop and implement personalized plans focusing on dysphagia, speech intelligibility, and cognitive‑communication needs.
• Provide interventions based on physician orders and educate caregivers in safe feeding techniques and communication strategies aligned with the care plan.
• Maintain detailed records per MDH and payer guidelines, ensuring accurate billing through proper CPT code usage and modifiers.
• Partner with the care team to review clinical progress, make adjustments to treatment plans, and ensure optimal patient outcomes.

• Master's degree in Speech Language Pathology.
• American Speech‑Language‑Hearing Association (ASHA) Certificate of Clinical Competence preferred.
• Experience with seniors, geriatric dysphagia, and home settings preferred.
• CPR certification.
