Sign‑On Bonus: $5,000
- $2,500 after 3 months of employment

- Follow the applicable dress code.
- Adjust personal schedule to meet facility needs when requested.
- Participate in clinical education activities for students/volunteers upon request.
- Attend/present at facility in‑service sessions as scheduled.
- Attend external professional growth activities as scheduled and communicate information to the team.
- Protect patient confidentiality at all times.
- Communicate professionally as evidenced by client satisfaction.
- Be familiar with and act according to applicable professional codes and the company handbook.
- Contribute to client relations and marketing activities.
- Attend and participate in community educational/promotional activities as requested.
- Makes positive contributions and promotes Progressive Care Therapy.
- Maintain licenses/certifications and memberships in applicable professional organizations.
- Provide age‑ and diagnosis‑appropriate therapeutic interventions as directed by the physical therapist.
- Evaluate patient response to treatment and provide feedback to the rest of the team.
- Document the course of patient care, including progress made and continuing need for treatment.
- Provide patient and family education on tasks and resources that will assist with continued functional independence.
- Therapeutic interventions result in positive outcomes as judged by patient satisfaction.
- Use positive feedback to promote patient participation in their rehabilitation program.
- Consult other team members to coordinate therapeutic activities.
- Include relevant subjective information when documenting patient response to treatment and functional changes noted.
- Evaluate patient response to the care plan, evidenced by patient achievement of goals or goal revision, and update the plan as needed.
- Provide information to patient/family/caregivers regarding current status, program planning, and potential discharge plans.
- Document interventions and effects at every session according to policy and procedure.
- Complete progress notes with objective data and goal progression.
- Provide input to the physical therapist regarding the discharge summary.
- Develop and review effective and appropriate Home Exercise Programs with each client to support independence prior to discharge.
- Measure effectiveness of education via family/client verbal feedback and patient satisfaction surveys.
- Coordinate treatment with other disciplines.
- Participate in rehab team meetings.
- Assist the physical therapist in providing team members with in‑service or cross‑training on physical therapy knowledge and skill areas as needed.
- Contribute to the development of improved efficiency and productivity of facility functions.
- Participate in interdisciplinary care conferences and provide/seeks input from others on patient care.
- Attend rehab meetings and offer ideas/solutions in problem‑solving situations or activities regarding the program.
- Attend in‑service sessions.
- Assist with leading in‑service sessions as requested or volunteered.
- Assist in orienting other staff to the physical therapy functions of the facility.
