Hybrid Housing Case Manager

Become a Housing Case Manager with The Mustard Seed in Grande Prairie, supporting clients in maintaining independent living through hybrid engagement. This role emphasizes community connections, daily living skills, and goal-oriented planning.

As a full-time Housing Case Manager, you will be part of a multidisciplinary team focused on empowering marginalized individuals. Key functions include fostering client independence, advocating for needs, and building rapport to enable clients to thrive. Your work will directly impact housing stability and access to community-based supports.

Key Responsibilities:

• Connect with clients to maintain permanent housing

• Advocate for clients and empower self-advocacy

• Attend client meetings and transport as needed

• Build rapport to support goal achievement

• Document client interactions and referrals

Requirements:

• Diploma or degree in a related field

• At least 1 year experience with marginalized individuals

• Strong communication and crisis management skills

• Valid driver’s license and vehicle insurance

• Commitment to ending homelessness in Alberta
