Hospice RN Case Manager

• Conduct ongoing assessments of the patient’s physical and psycho-social status.
• Collaborate with the Interdisciplinary Group (IDG), patient/family, and attending physician to develop a comprehensive care plan.
• Provide education and interventions to manage pain and other symptoms effectively.
• Ensure all necessary equipment, supplies, and medications are available for patient care.
• Adapt the IDG plan of care as the patient’s needs evolve.
• Document patient/family services accurately and in a timely manner.
• Participate regularly in IDG meetings to ensure coordinated care.

• Active, unrestricted RN/LPN license in the State of Michigan.
• At least one (1) year of relevant experience as an RN or LPN (required).
• Preferred:
  One (1) year of management experience.
• Preferred:
  One (1) year of home health experience.
• Current CPR Certification.
• Community health and a broad range of Medical-Surgical experience (preferred).
• Reliable transportation with a valid state driver's license and current car insurance.
