- Conduct ongoing assessments of the patient’s physical and psycho-social status.
- Collaborate with the Interdisciplinary Group (IDG), patient/family, and attending physician to develop a comprehensive care plan.
- Provide education and interventions to manage pain and other symptoms effectively.
- Ensure all necessary equipment, supplies, and medications are available for patient care.
- Adapt the IDG plan of care as the patient’s needs evolve.
- Document patient/family services accurately and in a timely manner.
- Participate regularly in IDG meetings to ensure coordinated care.
- Active, unrestricted RN/LPN license in the State of Michigan.
- At least one (1) year of relevant experience as an RN or LPN (required).
- Preferred:
One (1) year of management experience. - Preferred:
One (1) year of home health experience. - Current CPR Certification.
- Community health and a broad range of Medical-Surgical experience (preferred).
- Reliable transportation with a valid state driver's license and current car insurance.
