Community Care Givers – Grand Rapids, MI
Pay: $68–$70/visit
, based on Medicare home health and OASIS experience
Community Care Givers is seeking an experienced Registered Nurse (RN) to provide Medicare-certified home health services and complete Start-of-Care (SOC) admissions and OASIS assessments throughout the Grand Rapids area.
Responsibilities- Complete SOC, ROC, recertification, transfer and discharge visits as needed
- Perform comprehensive nursing assessments
- Develop and update individualized Plans of Care
- Complete medication reconciliation and patient education
- Coordinate physician orders and communicate with the care team
- Assess skilled nursing and homebound needs
- Maintain accurate documentation to support Medicare eligibility and medical necessity
- Current Michigan RN license in good standing
- At least 1 year of RN experience
- Medicare home health/OASIS experience strongly preferred
- Experience with Medicare SOC admissions preferred
- Strong clinical assessment, documentation and communication skills
- Reliable transportation and valid driver’s license
- Comfortable using electronic medical records and mobile documentation
Monday–Friday, primarily daytime hours
Some flexibility may be available based on experience and agency needs.
