Social Services Director - Overview

. The position is located at Royale Gardens Health & Rehabilitation Center in Grants Pass, OR. This role focuses on identifying and addressing the social, emotional, and psychological needs of residents, guiding them throughout their stay and discharge planning.

Responsibilities
  • Develop comprehensive social history assessment, psycho-social assessment, discharge plan, and complete MDS and Care Plan for new admits.
  • Orient and assist new residents in adjusting to the facility.
  • Assist residents and families with adjustment to changes, including room changes.
  • Assist families with the engraving and/or marking of residents’ personal items.
  • Review residents’ rights with residents and staff and attend to various personal needs of the residents.
  • Assist in the development of a volunteer program and facilitate family group meetings.
  • Document progress notes related to each resident’s care plan within policy time-frames.
Qualifications
  • Bachelor’s degree in social work or related field (preferred).
  • Health care experience highly preferred.
  • Meet any state and/or Federal requirements if applicable.
  • Basic understanding of computer technology.
  • Ability to read, write and follow oral and written directions in English.
  • Positive and cooperative relationship with residents, families, and employees.
