We are hiring a Registered Nurse to support our Grants Pass, OR patients.

$5,000 sign-on bonus (Full Time)

Pay Range: $53.00-$58.00/hr

Full time, Part time, and PRN opportunities available.

Responsibilities

The Registered Nurse (RN) in Home Health provides and directs nursing care to patients in their homes, as prescribed by physicians and in compliance with laws, regulations, and agency policies.

Coordinates the total plan of care with healthcare professionals, ensuring continuity of care by planning and exchanging information with physicians, agency staff, patients, families, and community resources—all within a Point of Care setting.

Conducts initial and/or comprehensive nursing evaluations, determines primary care focus, develops care plans within state guidelines, and submits accurate documentation in collaboration with physicians.

Performs comprehensive assessments at designated times and develops appropriate care plans.

Ensures patients meet home health eligibility and medical necessity guidelines as defined by the payer source.

Education and Experience

License Requirements:

• Current RN licensure in Oregon
• Current CPR certification
• Valid Driver's License, vehicle insurance, and access to reliable transportation
