RN/LPN Day/Night Shifts
Outfield Healthcare Partners is seeking experienced RN/LPNs to work day or night shifts in Grants, NM. This full‑time position offers competitive wages, raises and bonuses, as well as a comprehensive benefits package.
Job Type & ScheduleFull‑time. Days or nights available. Shift times: 6 pm–6 am or 6 am–6 pm.

We believe nursing is a work of the heart. In this role you will provide efficient and effective nursing care, ensuring positive clinical outcomes and resident/family satisfaction in accordance with accepted practice standards and regulations. You will work within the scope of practice defined by the state Nurse Practice Act, mentor staff, and collaborate with physicians and other medical professionals.
Principal Responsibilities- Conduct daily nursing functions in accordance with company, state, federal and local rules.
- Admit, transfer and discharge residents with proper documentation.
- Participate in orientation of new residents/family members.
- Participate in facility surveys and inspections.
- Ensure positive clinical outcomes and resident/family satisfaction.
- Serve as a resource to all staff regarding quality of services.
- Administer medications and treatments timely, following facility policy.
- Manage nursing personnel on shift to limit overtime and agency use.
- Assess work performance of nursing staff relative to unit standards.
- Conduct unit huddles to promote communication and provide updates.
- Participate in shift‑to‑shift communication.
- Coordinate shift assignments, resident assignments, and break times for CNAs.
- Collaborate with other departments to provide timely care.
- Assume administrative responsibilities for the center during weekends and holidays.
- Work beyond scheduled shift if needed to meet staffing needs.
- Ensure timely compliance and accuracy of the MDS process.
- Utilize Point Click Care (PCC) per company process.
- Ensure physician orders are followed.
- Carry out medication management to maintain adequate supplies.
- Notify physician/family of significant changes in resident health.
- Report incidents, accidents, adverse events promptly to Administrator and Director of Nursing.
- Investigate resident/family complaints and resolve them.
- Use infection control manual appropriately.
- Evaluate care interventions, identify problems, trends, and develop alternatives.
- Perform other duties as assigned.
- Graduate of an approved Licensed Vocational program and licensed in the state of practice.
- Minimum of 2 years of nursing experience in a Skilled Nursing Facility (preferred).
- Current BLS/CPR certification.
