Optum is seeking a Nurse Practitioner to join our House Calls team in Nevada, Placer, CA. Optum is a clinician‑led care organization that is creating a seamless health journey for patients across the care continuum.
As a member of the broader Home and Community Care team, you will provide annual clinical assessments to patients in the comfort of their homes. This preventive care helps identify and reduce health risks for patients, coordinate appropriate follow‑up care, and improve health and well‑being.
In this role you will have the ability to achieve work‑life balance with flexible scheduling offered to providers who can flex their time over the 7‑day work week. No on‑call, no weekends and no holidays are required.
Primary Responsibilities- Conduct in‑home assessments including past medical history, review of systems, medication reconciliation, vital signs, and comprehensive physical exam.
- Perform evidence‑based practice screenings including point‑of‑care testing (as appropriate).
- Identify diagnoses for care management and active medical management in the treatment plan.
- Formulate a list of current and past medical conditions using clinical knowledge and assessment findings.
- Communicate assessment findings to members’ PCPs to identify gaps in care.
- Identify urgent and emergent situations and intervene appropriately.
- Educate members on disease processes, medication, and compliance.
- Active NP license or ability to obtain by start date. Licensure must be unencumbered and free of any open disciplinary actions.
- Active ANCC or AANP national certification in Family, Geriatrics, Adult, Adult‑Gerontology Primary Care, or Adult‑Gerontology Acute Care, or ability to obtain by start date.
- Ability to complete physical requirements: moving a 30‑pound bag, navigating stairs, and performing physical assessment in various dwelling conditions.
- Reliable transportation to complete home visit assessments.
- 1+ years of clinical experience in the highest level of education, clinical setting.
- May be requested to obtain additional licensure in other geographic areas.
- Experience in gerontology, cardiology, internal medicine, or endocrinology.
- Home Health care or home visit experience.
- Ability to travel throughout the state, beyond the assigned region, up to 5% of the time. Eligible for additional compensation incentives with advanced notice.
- Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other language.
The salary range for this role is $104,500 to $156,000 annually based on full‑time employment. Salary is defined as total cash compensation actual range and pay mix of base and bonus is variable based on experience and metric achievement.
