HCRG Care Group is hiring a Band 3 Rehabilitation Assistant to join the Early Supported Discharge and Community Neuro Rehabilitation Team in Gravesham. You’ll help stroke and neurological patients regain independence, working at patients’ homes, in community settings and across rehabilitation environments.
You’ll collaborate with Therapists, Nurses and a multidisciplinary team, delivering personalised rehabilitation programmes and monitoring progress to support patients on their recovery
