Job Introduction

As a Band 5 Ward Nurse
, you will join our valued Neuro Stroke Team on Sapphire Ward at Gravesham Community Hospital. This is a rewarding opportunity to support patients recovering from stroke and neurological conditions, helping them regain independence and achieve their rehabilitation goals.

Main Responsibilities

You will contribute to the assessment, planning, delivery and evaluation of care for patients with stroke and a wide range of neurological conditions.

  • Deliver safe, effective and compassionate patient‑centred care
  • Contribute to the assessment, planning, implementation and evaluation of individual care plans
  • Support patients throughout their rehabilitation and recovery journey
  • Maintain accurate and contemporaneous patient records
  • Communicate effectively with patients, carers and the multidisciplinary team
  • Promote patient independence and well‑being wherever possible
  • Maintain a safe, clean and positive clinical environment
  • Work in accordance with NMC standards and professional guidelines
  • Provide flexible shift cover, including nights, weekends and bank holidays
Person Specification

Essential

  • Registered Nurse with a valid NMC registration
  • Passion for rehabilitation nursing and supporting patients to maximise independence
  • Excellent communication and interpersonal skills
  • Strong organisational and time‑management abilities

    Ability to work both independently and as part of a multidisciplinary team
  • Compassionate and patient‑focused approach to care
  • Confidence using IT systems including Microsoft Office

Desirable

  • Experience with in a rehabilitation, neurology, stroke or community hospital setting
  • Knowledge of caring for patients recovering from stroke or neurological illness
  • Experience working within a multidisciplinary environment
  • Commitment to continuous professional development
