You will work autonomously while collaborating closely with the wider multidisciplinary team to ensure patients receive timely and effective interventions. You will undertake comprehensive clinical assessments, develop personalised care plans and coordinate services to meet patient needs. You will be responsible for supporting patients with long-term conditions, frailty and admission avoidance pathways, ensuring care is tailored, proactive and responsive. Alongside your clinical responsibilities, you will provide leadership and support to nursing colleagues within the team.

You will help develop staff through supervision, appraisals, mentorship and clinical support, while contributing to service improvement, audit activity and quality initiatives.

Key responsibilities include:
