Are you a compassionate LPN or RN looking for a part‑time nursing role where your clinical skills and leadership truly make a difference? Alto Grayslake is seeking a dedicated Wellness Nurse to join our team across our Assisted Living and Memory Care neighborhoods.

• Employment Type: Part-Time
• Shifts Available:
  • 1st Shift | 7:00am - 3:00pm | 2 weekdays + every other weekend
  • 2nd Shift | 3:00pm – 11:00pm | 2 weekdays + every other weekend
• Pay: Starting at $34.00/hour

• Conduct resident health assessments and wellness checks across AL and Memory Care
• Administer medications and treatments per physician orders
• Supervise and support care staff in delivering person‑centered care
• Monitor residents for changes in condition and initiate appropriate interventions
• Coordinate with interdisciplinary team members, families, and outside providers
• Maintain accurate and timely clinical documentation
• Ensure compliance with state regulations and community care standards

• Active LPN or RN license in the state of Illinois (required)
• Current CPR certification (required)
• Experience in geriatric assisted living and/or memory care (preferred)
• Experience supervising care staff (preferred)
• Strong clinical assessment, communication, and organizational skills
• Compassionate, resident‑centered approach to care

Wellness Nurse

The Wellness Nurse supports the delivery of resident care and services in accordance with applicable regulations, accepted standards of nursing practice, and Allegro Living policies. This role provides clinical support, administers medications, monitors resident wellness, supports team member supervision, completes documentation, and communicates with residents, families, physicians, pharmacies, and outside providers.

The Wellness Nurse promotes a safe, supportive, resident‑centered environment while helping ensure timely documentation, quality outcomes, and compliance with Company policies and state requirements. The ideal candidate is compassionate, organized, collaborative, and committed to serving seniors with professionalism and sound nursing judgment.

• Provide daily supervision and guidance to assigned non‑licensed team members and other designated personnel in meeting residents’ needs.
• Monitor resident wellness, identify changes in condition, and respond appropriately to medication needs or other circumstances requiring additional care.
• Provide resident care in accordance with provider orders, regulatory requirements, recognized nursing practice, established standards, and Allegro Living policy.
• Accurately administer prescribed medications and support the continuity of resident care.
