This hybrid role requires two days per week on-site and reports to the Chief Operating Officer. The CQR Manager will implement compliance and quality processes across sites, ensure adherence to federal, state, and local regulations, maintain policies consistent with FQHC standards, and foster a culture of ethical conduct, patient safety, and continuous improvement.
Responsibilities- Compliance
- Implement and monitor the organization’s Compliance Program in alignment with HRSA, CMS, OIG, HIPAA, ACO, and UDS requirements.
- Maintain current knowledge of regulatory requirements and assist in updating policies.
- Conduct medical record audits following established processes.
- Manage the anonymous reporting hotline and perform investigations, ensuring timely documentation and follow‑up.
- Support corrective action plans and escalated findings to leadership.
- Quality
- Coordinate QA/PI activities including patient experience surveys and clinical quality measures (UDS, HEDIS, Annual Wellness Visits) pursuant to HRSA and PCMH requirements.
- Analyze and audit quality data to present to leadership and determine action plans.
- Provide education to departments on quality standards and initiatives.
- Risk Management
- Track incidents and assist with mitigation steps under established plans.
- Monitor incident reports, patient safety events, and trends to reduce organizational risk.
- Collaborate with clinical leadership to enhance patient safety and quality improvement initiatives.
- Survey Readiness
- Coordinate documentation and assist with readiness activities for audits and surveys.
- Monitor changes in federal and state regulations and communicate impacts to leadership.
- Education and Training
- Associate degree, bachelor’s preferred in business, healthcare, or related field.
- LPN or RN with knowledge of FQHC programs and other grant‑funded programs.
• Experience managing compliance programs and quality improvement initiatives in a healthcare setting.
• Knowledge of federal and state regulatory requirements (HRSA, CMS, OIG, HIPAA, UDS).
• Strong analytical and audit skills.
• Excellent communication and collaboration skills.
