Position: COMMUNITY CONNECTOR/Group Setting

Monday thru Friday (30 Hours Weekly) - Silver Spring, MD

Introduction: The Community Connector supports assigned individuals in a community-based day services model to promote independence and personal growth. The role involves ensuring integration into the community and creating meaningful daily activities based on each person's desires, strengths, and needs.

Key Responsibilities

Work with assigned individuals to implement their Individual Plan (I.P.), facilitate community integration, build skills, and develop personalized, goal-oriented daily schedules.

Engage individuals in meaningful community activities that foster skill development and social support for positive personal growth.

Inform supervisors and interdisciplinary team members of behavioral and daily issues that may arise.

Provide services as outlined in Individual Plans, ensuring goals are appropriate, implemented, and progress is tracked.

Demonstrate positive behavioral management techniques and communicate with individuals in a person-centered, age-appropriate manner.

Complete progress notes, daily logs, timesheets, mileage logs, and other documentation accurately and promptly.

Foster self-esteem through positive interactions and building trust with individuals.

Develop activities promoting community involvement, self-help, employment, and independent living.

Communicate with supervisors and individuals regarding schedule implementation.

Attend and participate in required training, staff meetings, stakeholder meetings, and supervision sessions.

Perform other duties as assigned.

Supervision

Supervised by the Assistant Director, Day Services.

Physical Qualifications

Flexible schedule required. Ability to stand/walk 50% of the time, sit and drive 50%. Capable of lifting, carrying, pushing, and pulling 20 pounds. Able to stoop, kneel, crouch, crawl, and reach. These physical abilities are necessary to assist and supervise individuals with disabilities.

Qualifications

High School Diploma or equivalent required. At least three years of experience working with people with disabilities or behavioral issues. Demonstrated ability in providing respectful support for at least one year. Bachelor's degree in special education, therapeutic recreation, psychology, or related field preferred. Use of personal vehicle required for transportation; must follow all Melwood transportation policies and maintain full liability insurance on the vehicle.
