As a Home Health RN, you will:

• Perform comprehensive skilled nursing assessments

• Complete Start of Care, Resumption of Care, Recertification, and Discharge visits as assigned

• Complete OASIS assessments as required

• Develop and update individualized plans of care

• Provide wound care and other skilled nursing treatments

• Perform medication reconciliation and provide medication education

• Provide disease-management education

• Monitor patients for changes in condition

• Communicate with physicians and obtain necessary orders

• Help identify concerns early and reduce avoidable hospitalizations

• Educate patients, families, and caregivers

• Coordinate care with the interdisciplinary home health team

• Complete accurate and timely clinical documentation using Well Sky

• Active Maryland Registered Nurse License Required

• Graduate of an accredited nursing program

• Reliable transportation

• Strong clinical assessment and critical-thinking skills

• Excellent communication and documentation abilities

• Ability to work independently in patients’ homes

• Home Health and OASIS Experience Preferred, but Not Required

• Well Sky Experience is a Plus
