Position OverviewWe seek experienced Host Home Providers willing to open their homes to individuals who can no longer live independently and prefer a private setting. Providers share their homes and lives, offering a private bedroom, hands-on support, and ongoing daily care. You can assist with medical concerns or behavioral challenges, making a meaningful difference as part of your family. Receive training on how to support these individuals to flourish.
Responsibilities- Assist with personal care, hygiene, meals, and dressing
- Offer employment coaching to help clients find jobs
- Organize recreational activities to promote social skills
- Include clients in family activities and outings
- Transport clients to appointments, activities, and outings
- Help with medical care, including appointments and medication management
- Document progress and medical records
- Train clients on life skills
- High school diploma or equivalent (Required)
- Pass criminal and motor vehicle background checks
- Complete training for Medication Administration, CPR/First Aid, and maintain ongoing training
- At least 1 year of experience as a host home provider
- Preferably experienced with behavioral needs and medical needs (e.g., CNAs, LPNs, RNs)
- Rewarding independent work

- Paid initial and ongoing training (Relias, Safety Care, CPR/First Aid, QMAP)

- Assistance with respite and home care aid recruitment
- Ongoing support, client programming, and events
