What You'll DoPerform comprehensive PT evaluations, admissions, and treatments for a wide variety of home care patient diagnoses — including post-surgical, neurologic, vestibular, and geriatric populations.

Deliver skilled, patient-centered physical therapy right in the comfort of patients' homes across the Greeley community and immediate vicinity.

Collaborate daily with a dynamic interdisciplinary team of nurses, PTs, PTAs, OTs, speech therapists, and medical social workers to deliver holistic, whole-person care.

Supervise Physical Therapist Assistants (PTAs) and serve as a trusted clinical resource and leader in the field.

Manage your caseload with autonomy and purpose — with a reasonable productivity expectation of just 28 points per week (5–6 visits per day), weighted thoughtfully for evaluations and admissions.

Leverage a full suite of clinical tools provided to you, including:

Work tablet & company phone

Ultrasound, e-stim & anodyne equipment

Theraband & additional therapy supplies at your discretion

Federal mileage reimbursement for every mile driven

You're a Great Fit If You...Hold a Physical Therapy degree and are licensed (or eligible for licensure) in Colorado Bring at least 2 years of PT experience in any clinical setting — or are a motivated new graduate ready to launch a meaningful career!

Have home health or home care experience (preferred, but absolutely not required — we invest in your growth!)Thrive working independently in the field while genuinely valuing team connection and collaboration

Possess excellent communication skills, adaptability, and strong clinical documentation abilities

Are comfortable and confident providing care across a wide variety of patient diagnoses and populations

Are passionate about setting patients up for lasting independence and success in their own space

Saturday
