Salary Range $28.00 - $34.00 Hourly

The Center at Center Place is recruiting for a Case Manager to assist with patient placement referrals, assessments, care plans, maintaining patient records, audits and other duties as assigned.

• Complete psychosocial assessments and maintain resident social histories.
• Coordinate discharge planning, including communication with hospitals, families, physicians, insurance providers, and community agencies.
• Assist residents and families with discharge needs, including home health, hospice, durable medical equipment, transportation, and community resources.
• Provide education regarding Medicare, Medicaid, insurance benefits, advance directives, and available financial assistance programs.
• Participate in interdisciplinary care conferences, resident care planning, and MDS meetings.
• Advocate for resident rights and assist in resolving resident and family concerns.
• Coordinate referrals to community resources and support services.
• Maintain accurate and timely documentation in Point Click Care  and resident medical records.
• Collaborate with nursing, therapy, admissions, dietary, activities, and other departments to ensure continuity of care.
• Monitor discharge goals and barriers, proactively working to facilitate safe and timely transitions.
• Assist with regulatory compliance, survey readiness, and quality improvement initiatives.
• Maintain knowledge of federal, state, and facility regulations related to resident care and discharge planning.
• Perform other duties as assigned.

Job Type: Full-time

Pay: $28.00-$30.00 per hour
