NP​/PA - Greeley

Gold PM&R is seeking a Nurse Practitioner or Physician Assistant to provide medical care for patients in inpatient rehabilitation and subacute settings. In this role, you will help manage patients’ medical needs during recovery and work closely with physicians, nurses, therapists, and facility staff.

• Perform patient evaluations and follow-up visits within your scope of practice.
• Assess and help manage acute conditions, chronic illnesses, and changes in clinical status.
• Review medications, laboratory results, and diagnostic findings; coordinate appropriate follow-up.
• Communicate with physicians, rehabilitation clinicians, specialists, patients, and families.
• Support treatment planning and transitions of care.
• Complete accurate, timely documentation in the facility’s electronic medical record.

• Graduate of an accredited Nurse Practitioner program.
• Current NP certification, as applicable, and active or obtainable authorization to practice in the state.
• Clinical experience and sound judgment in managing medically complex patients.
• Strong communication skills and comfort working with an interdisciplinary team.
• Inpatient, hospitalist, rehabilitation, or post-acute care experience is valued.

Facility-based daytime schedule, weekdays. Per-diem and part-time arrangements considered for the right clinician.

• Potential annual earnings of $150,000–$200,000+, depending on the opportunity.
• Inpatient practice with an anticipated daily patient volume of approximately 15–20.
• Schedule options may include Monday–Friday, 7-on/7-off, or weekends, depending on the facility.
• Employee and independent contractor opportunities may be available.
• Physician and specialist collaboration based on the facility’s care model.
• No in-house call where applicable.
