Position:
Registered Dietitian Nutritionist, East Mason, Green Bay, Wisconsin

Providing exceptional care for our patients and supporting co‑workers in a friendly, professional environment. Focus on patient needs, effective communication, time management, and high performance.

• Screen and assess patients' learning needs, monitor and reassess progress in a collaborative and ongoing manner.
• Conduct group and one‑to‑one educational sessions for patients and their families or support system using interactive, collaborative, skill‑based training methods for the pediatric through geriatric population.
• Provide clinical expertise and education to patients as part of the multidisciplinary education model used for these types of patients.
• Assist patients in developing educational and behavior change goals.
• Evaluate effectiveness of education by measuring attainment of learning objectives.
• Assist patients in developing individualized follow‑up plans and document education in the electronic medical record.
• Communicate and collaborate with other department staff and/or providers regarding patient care when appropriate.
• Refer patients to other services when needs are outside scope of practice.
• Collaborate with staff in development of educational training materials.
• Participate in continuous quality improvement processes.
• Assess personal strengths and weaknesses and develop a professional growth plan.

Assist with all aspects of care and services within a growing Endocrinology, Diabetes, and Nutrition Services practice, including telephone advice, serving as a nutrition resource to providers and community, communicating with hospitals and other offices, adhering to guidelines for clinical nutrition productivity, and utilizing appropriate computer functions.
