Position:
Licensed Practical Nurse (LPN) – Team Triage in Primary Care

Work hours:

34 hours per week (Full-time).

The Licensed Practical Nurse (LPN) is responsible for pre‑visit planning, standard rooming responsibilities, assisting with procedures, discharge instructions, and carrying out both administrative and clinical tasks as directed by a medical provider. This role is instrumental in helping patients feel at ease while collaborating with the provider throughout their visit.

• Room patients, perform and document vital signs and patient backgrounds.
• Coordinate provider schedules and appointments, enter orders and prepare patient charts.
• Instruct patients and families as directed within scope of licensure or certification.
• Assist medical providers with in‑office procedures and prepare/administer medications and immunizations.
• Maintain exam rooms and equipment to meet infection control standards.
• Ensure appropriate levels of medical supplies for each patient room.

• Current clinical experience as a licensed practical nurse.
• Knowledge of patient‑care principles and medical equipment.
• Awareness of safety hazards and precautions to maintain a safe work environment.
• Excellent interpersonal and communication skills.
• Ability to prioritize work, manage changing demands, and thrive in a fast‑paced environment.
• Ability to work under stressful or emergency situations with high concentration and energy.
• Proficient in EMR systems, computers, phone, and office equipment.
• Valid Practical Nursing license issued by the State of WI.
• Required CPR/Basic Life Support certification for healthcare providers.

Occasional sitting, frequent standing, walking, and driving. Occasional climbing (stairs/ladders). Occasional bent neck, frequent bent waist, frequent squatting.
