Position Summary

The Nurse Practitioner is responsible for quality care to patients being served at N.E.W. Community Clinic while collaborating with the multi-disciplinary team under the supervision of the Chief Medical Director.

Essential Job Functions
  • Obtain health history and perform physical examinations.
  • Assess patients to determine needs and develop a plan of care which is sensitive to the unique circumstances of patients and families experiencing homelessness.
  • Prescribe medication in accordance with Wisconsin statute and professional practice guidelines.
  • Perform minor surgical procedures.
  • Communicate referrals for patients requiring services not offered at N.E.W. Community Clinic taking into consideration available community resources and the financial challenges associated with our patients.
  • Review incoming diagnostic reports (e.g. lab, x-ray, EKG) in electronic medical record and follow-up in a timely manner.
  • Comply with the N.E.W. Community Clinic Infection Control plan while ensuring compliance by support staff.
  • Document patient care, services, and referrals using electronic medical record system.
  • Possess a thorough knowledge of the requirements for reporting communicable diseases in the State of Wisconsin and make appropriate referrals.
  • Demonstrate adaptability in responding to changing trends in nursing and share knowledge and experience.
  • Communicate effectively with patients, staff, outside agencies, students, volunteers, and other visitors to the clinic.
  • Protect confidentiality as directed by HIPPA regulations.
  • Maintain regular clinical review meetings with collaborating physician.
  • Work effectively as a member of the health care team.
  • Carry out professional nursing role as described in the Wisconsin Nurse Practice Act and the American Nurses Association Social Policy Statement.
  • Maintain professional continuing education certification requirements and unencumbered professional license.
  • Flexibility to work at other sites within the N.E.W. Clinic and outside facilities as needed.
  • Able to work well with a diversified population and cultures with varied lifestyles.
  • Excellent interpersonal and communication skills.
  • Able to work as part of a team with minimal supervision.
Other Responsibilities
  • Assist clinic leadership in preparation of yearly program goals and objectives.
  • Provide supervision to the clinic staff as needed when the clinic leaders are present or absent.
  • Participate in clinic staff meetings as required by clinic leadership.
  • Transport patients on a limited basis.
  • Participate and work with clinic leadership in accepting students from various school programs to shadow in the clinic as needed.
Education
  • Obtain Masters or Doctoral degree in Nursing or related field.
  • Completion of clinical pharmacological education.

Experience
  • Experience in the role of Advanced Practice Nurse Practitioner a plus.
  • Experience and understanding of diverse cultural groups in the community.
  • Valid driver's license, registration, auto insurance, and automobile.
Language
  • Comprehend and use basic language either written or spoken to communicate information and ideals.
  • Ability to speak a second language a plus.
Hours Of Work
  • May vary based on Organizational need.
Travel
  • May vary based on Organizational need.

Salary: $52.00 - $63.00 per hour
