The RN Care Manager will oversee care management and service coordination, ensuring quality, person-centered care in line with member needs.
Qualified candidates must have a current RN license in Wisconsin and preferably a Bachelor's degree in nursing. Experience with skilled nursing, particularly in gerontology or home care, is highly desirable. The position involves in-home visits and requires a valid driver's license.
