Med Star Health is looking for a PRN Home Health Aide to join our team at Med Star Health Home Care, providing care in patients' homes in Chevy Chase, Bethesda, Kensington, Takoma Park, and Silver Spring territories. The ideal candidate will have excellent communication skills and a passion for in‑home care.

• Attend agency regional office and team meetings as required; attend in‑service educational programs.
• Demonstrate flexibility in adjusting to changes in procedures and daily schedules.
• Maintain effective working relationships with other departments and participate in multidisciplinary quality and service improvement teams.
• Participate in meetings and on committees, representing the department and agency in community outreach efforts.
• Clean direct patient care areas as necessary; identify safety hazards in the home and report them to the Operations Director or primary case‑manager.
• Assist with transfers and range of motion exercises, perform personal care activities (bathing, dressing, oral hygiene, etc.), and light meal preparation and laundry as instructed.
• Document services in accordance with agency medical records and time‑keeping policies, and prepare clear patient progress reports for multidisciplinary case conferences.
• Recognize signs of emotional distress and encourage patient mobility and socialization; respond to special needs of patients/families experiencing terminal illness.
• Report changes in patient behavior or mental status, care or family health or social problems, and any obstacles in carrying out assigned activities to the case manager or Operations Director.
• Collaborate with case managers or professional staff to revise and implement the Home Health Aide plan of care and inform Operations Director of any needed schedule changes.

• Ability to read, write, and perform arithmetic calculations.
• Certified Nursing Assistant (CNA) in the District of Columbia, Maryland, or Virginia.
• Geriatric Nursing Assistant (GNA) in the District of Columbia, Maryland, or Virginia.
• Certified Home Health Aide (CHHA) in the District of Columbia, Maryland, or Virginia.

This position has a hiring range of: USD $19.55 - $34.25 /hr.
