Job Overview
Licensed Practical Nurse (LPN) at Integritus Healthcare
. This is a full-time position with a flexible schedule, weekly pay, and mileage reimbursement. Pay range: $35.00 - $39.04/hr (based on experience).
This role is responsible for providing direct patient care under the supervision of the Registered Nurse. Responsibilities include following the plan of care, delivering treatments, and collaborating with the care team to help achieve positive patient outcomes.
Responsibilities- Provides direct patient care within the parameters of the current nursing license.
- Implements current nursing practice by following the patient assessment and approved plan of care initiated by the RN Case Manager.
- Completes, maintains, and submits accurate and relevant clinical notes about the patient’s condition and care. Records pain and symptom management changes and/or outcomes as appropriate.
- Performs Point of Service documentation as part of quality care, accuracy, and communication standards; documents timely and communicates with the Patient Care Team.
- Assesses and provides patient and family/c caregiver education and information pertinent to diagnosis and self-plan of care.
- Participates in the coordination of hospice services and reports identified needs to hospice interdisciplinary team members.
- Uses and prepares equipment and supplies appropriately, adhering to aseptic technique.
- Provides appropriate pain and symptom management and evaluates patient response to treatments and medications.
- Participates in personal and professional growth and development.
- Participates in the organization’s quality management program.
- Participates in on-call schedule, including evening, night, holiday, and weekend rotations to ensure 24-hour coverage.
- Other duties as assigned.
Minimum qualifications (unless stated otherwise):
- Experience:
Minimum of one (1) to two (2) years in health care; three (3) years preferred. Hospice experience and acute care in an institutional setting are preferred. Home care experience is preferred. - Education:
Graduate of an accredited Practical Nurse or Vocational Nursing program. - License/Certification:
Current Massachusetts LPN/LVN licensure; CPR certification; valid driver’s license. - Other:
Understanding of hospice philosophy and issues surrounding death/dying.
- May be exposed to extreme temperatures and must drive in various weather conditions.
- Exposure to infections and contagious diseases; may work with patients in a wide range of circumstances and emergencies.
- Travel required. OSHA exposure categories may apply (Category I–III as appropriate).
