Licensed Practical Nurse

CHD is seeking a full-time LPN to work flexible business hours Monday-Friday, no on-call. The LPN would become part of a wonderful team of nurses that work with our behavioral health outpatient clinic in Greenfield, MA. The LPN will oversee clinic triage, medication refill requests, lab review, prior authorizations, and collaborating with the client's care team, other medical providers, and other CHD programs.

ESSENTIAL JOB FUNCTIONS

CLIENT SERVICES

• Develop strong, supportive relationships with clients
• Implementation of client medication treatment plans
• Interacting with clients in order to assess needs, provide education and information around health issues and the role of prescribed medications in their treatment
• Assistance in the management of medical issues
• Coordination of clients’ medical treatment with other service providers

PROGRAM AND STAFF DEVELOPMENT SERVICES

• Participation in team approach including communication with Program staff
• Ensuring adherence to established CHD policies and procedures for client treatment
• Consultation with Program staff for coordination of client treatment
• Attendance and Participation in weekly staff meetings
• Attendance and Participation in monthly meetings with the psychiatry team

MINIMUM QUALIFICATIONS

• Licensed Practical Nurse in good standing
• Two years experience working with a psychiatric population preferred
• Valid driver’s license and registered vehicle

WORK LOCATION

This position is an office position (M-TH) with one day telecommuting available on Friday. The office is located in Greenfield, MA

$68,000 - $78,000
