Life Path in Greenfield, MA is seeking a full-time Home Care Case Manager (35 hrs/week) based in our Greenfield office serving Franklin County and North Quabbin area.
The Case Manager develops supportive relationships with older adults, coordinates Home Care services, assesses needs, and refers clients to resources. Strong communication and documentation skills are required; bilingual abilities qualify for a pay differential after fluency assessment.
