Job SummaryThe Clinical Manager coordinates and oversees all direct care patient services provided by clinical personnel. The role includes managing assignments, developing clinical plans, ensuring compliance with regulatory agencies, supervising staff, participating in sales and marketing initiatives, and acting as the Branch Director in their absence. The position requires strong leadership, communication, and clinical knowledge.
Responsibilities- Develop, plan, implement, analyze, and organize clinical operations for a specific location.
- Conduct and delegate assessments and reassessments of patients, updating care plans and interpreting patient needs.
- Manage assignment of caregivers.
- Ensure delivery of care to all patients, receive and review case referrals, determine admission criteria, assign appropriate clinicians.
- Instruct and guide clinicians to promote effective performance and quality home care services.
- Assist clinicians in establishing therapeutic goals and developing patient Plan of Care.
- Monitor cases to ensure documentation compliance and complete billing in accordance with Medicare regulations.
- Coordinate communication between team members, physicians, and caregivers.
- Work with the Branch Director and Finance Department to establish revenue and budget goals.
- Participate in sales and marketing initiatives.
- Supervise all clinical employees assigned to a specific location, evaluating performance and providing coaching.
- Handle employee corrective actions and discipline issues in consultation with HR.
- Recruit, interview, hire, train, and develop direct care clinicians.
- Assess staff education needs, develop, and conduct staff education sessions.
- Analyze situations, identify problems, and evaluate alternative courses of action.
- Review case managers and clinical staff documentation for quality and compliance.
- Perform QA/PI activities and collaborate with Utilization Review staff for data tracking and outcomes analysis.
- Provide direct patient care infrequently and in emergencies.
- Act as Branch Director in their absence.
- Interpret company policies to ensure compliance with external regulatory authorities and caregiver documentation standards.
- Participate in performance improvement activities and maintain clinical knowledge through training.
- Maintain relationships with referral and community sources.
- Perform other related duties as assigned.
- Associate degree in Nursing (ADN) required;
Bachelor of Science in Nursing (BSN) strongly preferred. - Active and unrestricted Registered Nurse (RN) license or Compact State RN license with authorization in North Carolina.
- Proof of current CPR card and/or BLS certification.
- Valid driver’s license, auto insurance, and reliable transportation.
- A minimum of 2 years of experience as a Registered Nurse.
- Home health experience preferred.
- Management and people leadership experience strongly preferred.
- OASIS experience preferred; OASIS certification (HCS‑O, COQS, or COS‑C) preferred.
- Homecare Homebase (HCHB) experience preferred.
- Foundational knowledge of CMS PDGM preferred.
- Normal Hours of Operation:
Monday–Friday, 8:00 a.m.–5:00 p.m. (ET). - Weekly
