Job SummaryAssess and identify patient/caregiver problems, establish nursing patient/caregiver care goals, and identify and implement interventions for all agency programs. Document outcomes of care. Work with physicians to establish and implement a medical plan of care for the patient. Work closely with other members of the interdisciplinary group and communicate effectively both verbally and in writing with group members, patients, caregivers, physicians, pharmacists, and other health care professionals and community service organizations.
Establish and maintain a therapeutic relationship with the patient/caregiver, including providing emotional support and teaching.
- Graduate of an accredited School of Nursing.
- Currently licensed as an RN in North Carolina and in good standing with NCBON.
- One year of experience in an acute, hospice, home health, or long‑term care setting.
- CPR certification from the American Heart Association or American Red Cross Basic Life Support (BLS).
- Valid state‑issued driver's license and automobile liability insurance at limits required by the agency; must have own transportation.
