PRN RN/LPN Home Care - Hospice (Greensboro & Burlington, NC) – Authora Care Collective

Manage the overall plan of care for a caseload of hospice patients in collaboration with the interdisciplinary team. Assess and identify patient/caregiver problems, establish nursing goals and interventions, and work with physicians to develop a medical plan of care. Build and maintain a therapeutic relationship with patients and caregivers, provide emotional support, education, and dietary counseling as needed. Communicate effectively with all team members and document outcomes.

Assume on‑call responsibilities as scheduled.

• Collaborate with interdisciplinary group (IDG) to coordinate care plans.
• Assess patient and caregiver needs and set nursing goals.
• Implement interventions and monitor progress.
• Work with physicians to establish and update medical care plans.
• Provide emotional support, teaching, and dietary counseling.
• Document care outcomes accurately.
• Assume on‑call responsibilities as scheduled.

• Graduate of an accredited School of Nursing.
• Current RN/LPN license in North Carolina with good standing in NCBON.
• CPR certification from the American Heart Association or the American Red Cross Basic Life Support (BLS) (required).
• Experience in an acute, hospice, home health, or long‑term care setting (preferred).
• Valid state‑issued driver’s license.
• Own transportation and automobile liability insurance at agency‑required limits.
