Responsibilities:
- Administer skilled nursing care to patients in their place of residence under the supervision of an RN.
- Assist in planning, coordinating, and implementing patient care in collaboration with the interdisciplinary team.
- Provide education to patients and families on self-care techniques and assess their response.
- Document care interventions accurately and communicate effectively with patients, families, and healthcare professionals.
- Support end-of-life care duties and help with patient admissions, discharges, and transfers.
