Care Manager - Emergency Department at ECU Health
Position Summary:
The Care Manager coordinates the plan of care through collaboration with the provider, interdisciplinary team, patient and family. The role enhances patient care quality, optimizes clinical outcomes, manages cost of care, and identifies performance improvement areas.
- Manage, coordinate, and facilitate patient care progression throughout the continuum.
- Complete psychosocial assessments using departmental procedures within required time frames.
- Collaborate and maintain active communication with providers, nursing, and other members of the health‑care team to effect timely, appropriate patient management.
- Utilize critical thinking, assessment findings, and clinical skills to lead the interdisciplinary team in plan of care development and implementation.
- Identify delays or alterations in the plan of care and address system problems impeding diagnosis or treatment to ensure delivery of care in the most appropriate setting.
- Make necessary arrangements to facilitate and coordinate support services to significantly improve outcomes through safe, sustainable plans of care and appropriate follow‑up.
- Identify and apply information about patient’s health behaviors, cultural influences, and belief or value system; re‑assess based on potential barriers and work with the health‑care team to address them.
- Education:
Associate Degree in Nursing or higher or Bachelor of Science in Social Work or higher. - Registered Nurse: current licensure by the NC Board of Nursing is required.
- Experience:
0‑3 years of medical case management.
- Shifts:
Thursday, Friday, Saturday, Sunday 7 a.m. – 5 p.m. (4‑10 hour shifts). - Unit:
Emergency Department.
