Case Manager

CLINICAL SUPPORT POSITION – NOT SUBJECT TO STATE HUMAN RESOURCES ACT.

The Case Manager – Patient Navigator is part of the Brody School of Medicine HIV Program (BSOMHIVP) to link and retain people living with HIV/AIDS (PLWHA) in care. The role provides a range of client‑centered services to retain PLWHA who are in care at BSOMHIVP by supporting newly diagnosed individuals and those who have had a lapse in health care. The position also supports clients in assessing and navigating the new federal health‑care ‘insurance exchange’ system in North Carolina.

This grant‑funded role is within the Division of Infectious Diseases at East Carolina University, Department of Medicine. The Medical Case Manager is primarily a self‑directed position and will work in collaboration with other members of the ECUHIVP treatment adherence team to link and retain PLWHA in HIV care. The employee will work under the direct supervision of the ECUHIVP Ryan White program administrator.

Work activities are both individual and community based.

• Assess clients for barriers to HIV care and develop action plans to address identified barriers, monitor care plan objectives, and assist clients with accessing medical services, mental health, and substance abuse services.
• Educate clients on HIV disease and encourage active engagement in their care.
• Identify clients who have fallen out of care, track these clients, and encourage re‑engagement in care.
• Coordinate support services that help clients remain in care.
• Assist clients in completing the Ryan White eligibility process and reschedule provider appointments for clients who miss appointments.
• Enrol clients in affordable HIV medication programs such as the HIV medication assistant program, 340B medication program, and drug company prescription assistance programs.
• Work flexible hours, including possible nights and weekends, as required by the nature of the job.

Employment in this position provides eligibility to participate in the UNC Optional Retirement Program (ORP), which offers matching employer contributions and employee control of investment choices, along with eligibility to participate in various Voluntary Supplemental Retirement Plans (401(k), 403(b), 457). This position is not eligible to participate in the NC Teachers’ and State Employees’ Retirement System (TSERS). While this position is not automatically eligible for the NC Teachers’ and State Employees’ Retirement System (TSERS), new hires transferring from another North Carolina state agency and current ECU employees transferring internally who had been enrolled in TSERS in their previous employment will continue participation in TSERS. These individuals may be able to elect to participate in ORP if they had not been offered that option at the time of their initial enrollment into TSERS.
