The hospital provides acute, intermediate, rehabilitation, and outpatient services to a 29‑county region of more than 1.4 million people.
Position Summary- Coordinates hospital‑to‑hospital transfers following state and federal regulations.
- Interprets diagnoses and specialized patient‑care needs.
- Acts as a hospital and physician resource and maintains compliance with Access to Care laws.
- Provides follow‑up to referring practitioners and facilities regarding referral status time frames.
- Performs initial utilization‑management screening.
- Coordinates and facilitates all admission/bed requests from inpatient/outpatient/clinics/pre‑admits/regional referrals for cardiovascular services, following established algorithms, protocols, guidelines, and procedures.
- Documents special requests or conditions of transfers and develops plans of how, when, and where to proceed with patient placement requests.
- Balances external referral requests with internal ED and transfer needs.
- Uses Web On‑Call physician call schedules to identify coverage for unassigned patients.
- Uses the IWARN system to communicate internally and externally with physicians, Cath Lab on‑call staff, and referring facilities.
- Evaluates the needs of medicine and surgery bed requests with respect to the system and regional referral list, providing direct communication with referring and accepting clinicians.
- Communicates and collaborates with physicians, clinicians, internal departments, and referring facilities regarding clinical screening, referrals, placement, facsimiles, bed assignments, and patient acceptance.
- ADN required; BSN preferred.
- RN license required.
- American Heart Association BLS and ACLS certifications preferred.
- At least five years of experience in a patient‑care environment utilizing clinical and assessment skills.
- Charge nurse experience required.
- Knowledge of admission, patient placement, transfer, and discharge processes preferred.
- EPIC user.
- Teletracking user experience is a plus.
- Utilization management experience is a plus.
Shift: Nights 7 pm to 7 am.
