Registered Nurse - RN – Care Center

• One day per week, with every third weekend rotation

PM Shift – Full-Time

• Schedule:
  
  6:00 p.m. – 6:30 a.m.
• Three days per week, with every third weekend rotation

The Care Center RN provides direct patient care in our Inpatient Care Center, ensuring compassionate, high-quality nursing for patients and families. This role focuses on skilled symptom management, emotional support, and patient education as part of a collaborative hospice care team.

Deliver skilled nursing care to patients and families, including:

• Inpatient Care Center
• Referrals and admissions
• Chart reviews and other assigned duties
• Follow established Plan of Care and physician orders; communicate changes as needed
• Coordinate and participate in IDG planning and care review
• Teach and supervise hospice aides, volunteers, and primary caregivers
• Ensure smooth transitions between home care and inpatient care
• Educate patients and families on hospice services and benefits
• Manage physician orders and durable medical equipment (DME) requests
• Complete all required documentation accurately and on time
• Maintain clinical competence and provide weekend/evening support as needed

• Graduate of an accredited School of Nursing
• Current RN license in the State of Ohio
• Minimum 2 years of experience in Medical/Surgical, Oncology, or Community Health Nursing
• Hospice experience preferred

• Valid driver’s license
• Strong customer service and communication skills
• Excellent attention to detail, organization, and documentation skills
• Ability to work independently and follow instructions
• Willingness to travel to other sites as needed (including in inclement weather)
• Able to lift up to 25 lbs and assist with patient mobility (ambulation, transfers, positioning, etc.)
