The Primary Care Center Nurse is responsible for direct patient care in the Inpatient Care Center. This position is 6a-630p, one day a week with every third weekend. Additional hours are typically available for pick up.

Education and Experience
  • Graduation from an approved School of Nursing.
  • Current RN licensure in the State of Ohio.
  • Minimum of two years of experience in Medical/Surgical, Oncology or community health nursing.
  • Hospice experience preferred.
Special Skills and Requirements
  • Valid driver’s license.
  • Detail oriented with excellent communication, documentation, and organizational skills.
  • Ability to follow instructions.
  • Ability to work independently without supervision.
  • Travel to other sites as needed. Travel may be in inclement weather.
  • Ability to lift 25 lbs of supplies or equipment.
  • Requires various amounts of sitting, standing, twisting, turning, bending, lifting and walking.
  • Ability to assist with patient care activities such as ambulation, transfers, and positioning.
Responsibilities
  • Provide competent nursing care to assigned patients through skilled symptom management, emotional support, appropriate teaching and timely response to patient and family needs including:
    • Inpatient Care Center
    • Referrals and Admissions
    • Chart Reviews
    • Other duties as assigned.
  • Provide patient/family services according to established Plan of Care and signed physician orders. Consult patient’s physician and/or Medical Director when necessary and initiate appropriate telephone orders for changes in Plan of Care.
  • Coordinate IDG planning for assigned patients.
  • Participate in the IDG analysis of the patient resulting in the development, implementation, and revision of the IDG Plan of Care.
  • Teach, supervise, and evaluate the care provided by volunteers, hospice aides, and primary caregivers.
  • Facilitate continuity of care by coordinating transfer/admission of home‑care patients to inpatient setting.
  • Facilitate continuity of care by coordinating transfer/admission of inpatient patients to home‑care setting, if applicable.
  • Explain hospice services and benefits in regards to Medicare, Medicaid and private insurance and know when to use Palliative Care Interventions and ABN’s.
  • Obtain and enter orders from Medical Directors and Physicians when deemed appropriate.
  • Order appropriate Durable Medical Equipment (DME) for patients.
  • Perform competent initial nursing assessments.
  • Work evenings and weekends as needed to meet the needs of patients and families and agency backup schedule.
  • Complete all documentation as required by the organization in compliance with Agency policy, accreditation bodies, and governing requirements.
  • Maintain clinical competency for assigned responsibilities and procedures.
  • Other duties as assigned.
