Position: Licensed Practical Nurse (LPN)

Schedule: Monday–Friday, 8:00 AM – 4:30 PM

Holiday Rotation: 2 holidays per year

Hours: Full-time (40/week)

Summary of Position: The Licensed Practical Nurse (LPN) delivers high‑quality, compassionate direct patient care in the home, community, and care‑center settings. This role supports patients and families through skilled symptom management, emotional support, and exceptional service aligned with the organization’s mission of providing dignified, patient‑centered hospice care.

Key Responsibilities
  • Provide compassionate, high‑quality hospice and palliative nursing care through home visits, inpatient care, end‑of‑life/continuous care, and admissions support
  • Deliver care in accordance with individualized Plans of Care and physician orders
  • Monitor patient condition and promptly communicate changes to the supervising RN
  • Educate patients and families on hospice services, insurance benefits, and care expectations
  • Coordinate care transitions and order necessary Durable Medical Equipment (DME)
  • Obtain and accurately document physician orders in the EMR
  • Complete timely, compliant documentation per agency and regulatory standards
  • Collaborate with interdisciplinary team members to ensure seamless, patient‑centered care
  • Participate in evening and weekend rotations as scheduled
  • Perform additional duties as assigned to support patient care and team operations
Qualifications
  • Minimum 2 years LPN experience (Med/Surg, Oncology, or Community Health; hospice preferred)
  • Valid driver’s license and reliable transportation
  • Strong communication and time management skills
  • Ability to work independently
  • Medical, dental, and vision eligibility 1st of the month following hire
