Includes rotating every 4th weekend and holiday rotation.

The After-Hours Nurse is responsible for direct patient care after business hours.

• Graduation from an approved School of Nursing.
• Current RN or LPN licensure in the State of Ohio.
• Minimum of two years of experience in Medical/Surgical, Oncology or community health nursing.
• Hospice experience preferred.

• Shall have a valid driver’s license.
• Detail oriented with excellent communication, documentation, and organizational skills.
• Ability to work independently without supervision.
• Travel to other sites as needed. Travel may be in inclement weather.
• Ability to assist with patient care activities such as ambulation, transfers, and positioning.

• Provide competent nursing care to assigned patients through skilled symptom management, emotional support, appropriate teaching and timely response to patient and family needs, including:
  • Routine homecare visits
  • End‑of‑life/continuous care
  • Referrals and admissions
  • Care center assistance
  • Chart reviews
  • Other duties as assigned
• Explain hospice services and benefits regarding Medicare, Medicaid and private insurance, and know when to use palliative care interventions and ABNs.
• Obtain and enter orders from Medical Directors and Physicians when deemed appropriate.
• Order appropriate Durable Medical Equipment (DME) for patients.
• Facilitate continuity of care by coordinating admission of home‑care patients to in‑patient settings (hospital, nursing home), as needed.
• Complete all documentation per organization policy, accreditation bodies and governing requirements.
• Other duties as assigned
