KEY RESPONSIBILITIES:

* Responsible for taking and processing Palliative Care referrals.

* Provide support to the Palliative Care Nurse Practitioner and Medical Director.

* Will schedule visits to support goals of care as ordered by MD.

* Coordinate and communicate patient care goals between all providers.

* Make calls to present the palliative program, obtain legal consents, and assess interdisciplinary needs.

* Ensure communication of assessment and reassessment findings to attending physician.

* Complete all documentation accurately, timely, and in accordance with company policy and procedure.

* Inform palliative team of all new admissions and discharges.

* Coordinate referrals utilizing the referral process.

* Serve as a liaison for medical directors and nurse practitioners, enhancing effectiveness and compliance.

* Run billing report and address validation holds/ billing errors

LICENSURE, CERTIFICATION, AND

EDUCATIONAL REQUIREMENTS:

* Current, active, and unrestricted Registered Nurse (RN) OR Licensed Practical Nurse (LPN) licensure in the state of practice

* A minimum of three (3) years of nursing experience

* Palliative care, hospice, or home health care experience is preferred

* CHPN (Certified Hospice and Palliative Nurse) is preferred
