Position Summary:We are seeking a motivated Occupational Therapist (OT) to join our home health team. In this role, you will assess functional needs and deliver in-home occupational therapy services to adults and older adults with a variety of physical, cognitive, and environmental challenges. Your interventions will help patients achieve greater independence in their daily lives.
Key Responsibilities:- Conduct initial assessments and create customized plans of care for each patient
- Deliver evidence-based occupational therapy interventions for ADL/IADL limitations, home safety, fall risk, and cognitive function
- Collaborate with physicians, nurses, physical therapists, speech therapists, and care coordinators
- Provide family and caregiver training on therapy goals and safety strategies
- Document evaluations, progress notes, and reassessments in the EMR system (Your Tailor Made Senior Services)
- Update care plans regularly based on patient progress and changing needs
- Comply with agency policies, Medicare regulations, and professional standards of care
- Supervise Occupational Therapy Assistants (COTAs) as needed
- Participate in interdisciplinary meetings and case conferences
- Promote occupational therapy education in the community, if applicable
- Current Occupational Therapy license in the State of Texas (Required)
- Graduate of an ACOTE-accredited Occupational Therapy program
- At least 1 year of OT experience (Home health or geriatrics preferred)
- Strong clinical judgment and independent decision-making skills
- Reliable transportation, valid driver’s license, and proof of insurance
- Comfortable using EMR and mobile documentation tools
- Flexible scheduling (Full-Time, Part-Time, or PRN)
- W-2 or 1099 options available
