• Evaluate patient and home health environment and perform skilled home health assessments; complete OASIS documentation and visit notes; develop care plans; communicate with physicians, the care team, patients, and families.
• Develop the plan of care based on findings from the evaluation visit.
• Serve as the patient advocate and maintain ongoing communication with physicians, referral sources, caregivers, and facility management to facilitate coordination of care.
• Administer medications and treatments as prescribed by the physician. Provide direct patient care including infusion therapy, wound care, teaching, and disease management.
• Provide direct supervision of nursing clinical care team members (LVNs & HHAs).
• Participate in regular case conferences and in-services.

• Strong organization skills and time management
• One year of previous geriatric home health experience required
• Able to multitask and meet deadlines
• Professional appearance and effective communication skills
• Current Texas Registered Nurse (RN) license
• One year of clinical nursing experience
• Current Texas driver's license
• CPR Required
• Experience with healthcare software (Kinnser preferred)
• One year of previous OASIS experience
• Competency to perform any skilled procedure within the Nurse Practice Act
• PICC Line and Wound Vac Experience (preferred)
