Job Description 3 months contract with local authority

We are looking for an experienced Locum Social Worker to join our Hospital Discharge Team. This role is crucial in facilitating safe and timely discharges through the Discharge to Assess (D2A) process. You will collaborate with a multidisciplinary team to conduct Care Act assessments, Mental Capacity Assessments, and develop strengths-based care plans for adults transitioning out of the hospital. Your responsibilities will include coordinating discharge arrangements across D2A pathways (1, 2, and
3) to ensure patients receive appropriate support while their long-term needs are evaluated outside the hospital setting.

Responsibilities
  • Facilitate safe and timely hospital discharges following D2A pathways and local discharge policies.
  • Conduct Care Act assessments, Mental Capacity Act assessments, and make Best Interest decisions as needed.
  • Develop person-centered discharge plans and coordinate community support, reablement, or short-term placements.
  • Collaborate with multidisciplinary team members, patients, and families to ensure effective discharge planning.
  • Identify and address safeguarding concerns in accordance with statutory duties.
  • Maintain accurate and timely case records while managing a fast-paced caseload.
Requirements

Requirements:

  • Qualified Social Worker registered with Social Work England.
  • Experience in hospital discharge, reablement, or adult social care.
  • Strong knowledge of the Care Act, Mental Capacity Act, D2A pathways, and safeguarding procedures.
  • Ability to work independently and effectively within a multidisciplinary team environment.
