Travel Physical Therapist (PT) – 13 Weeks
Shift: Days | Mon–Fri (make-ups on weekends if needed)
Setting: Outpatient (≈20% Assisted Living)
Length: 13 weeks | GT: 36 hrs/week
Patients: 6–7/day
Charting: Well Sky (Home Care/Home Base experience required)
Travel Radius: 50 miles | Mileage: Included (≈50–75 miles/week)
Requirements:
Active PT license
2+ years recent PT experience
1 completed travel assignment required
Home health/Home Base experience (required)
BLS & First Aid
No new grads
Additional Info:
Daily stand-up meetings at 8:30 AM (Mon–Fri)
Professional scrubs & closed-toe shoes
COVID vaccination & boosters required (exemptions case-by-case; weekly testing if approved)
Housing must be secured prior to submission
