Overview

Position Summary: The Home Health Wound Nurse is a registered nurse (RN) responsible for providing specialized wound care to patients in their homes, ensuring proper treatment, and promoting healing. The role requires assessment, planning, implementation, and evaluation of wound care interventions in collaboration with the healthcare team. The Wound Nurse will work closely with physicians, family members, and caregivers to develop and follow individualized care plans to support optimal patient outcomes.

Responsibilities
  • Wound Assessment & Management:
    • Conduct thorough wound assessments for new and ongoing cases, including evaluation of wound type, stage, and severity.
    • Create and implement individualized wound care plans based on assessment findings and physician orders.
    • Administer prescribed treatments, including dressing changes, debridement, and application of topical agents.
    • Monitor and document wound healing progress, adjusting care plans as necessary.
  • Patient and Family

    Education:

    • Educate patients and caregivers on wound care techniques, infection prevention, and the importance of adherence to the prescribed care regimen.
    • Provide guidance on proper nutrition and lifestyle modifications to promote wound healing.
    • Offer emotional support to patients and families regarding wound care and recovery.
  • Collaboration with Healthcare Team:
    • Work closely with physicians, physical therapists, and other healthcare providers to ensure comprehensive and coordinated care.
    • Participate in care plan development and interdisciplinary team meetings to discuss patient progress and adjust treatment strategies.
    • Provide feedback to the clinical supervisor regarding patient progress and potential changes to care plans.
  • Documentation and Compliance:
    • Accurately document wound assessments, treatments, and patient responses in the electronic health record (EHR).
    • Maintain up-to-date records of wound care interventions, patient outcomes, and follow-up plans.
    • Ensure compliance with regulatory guidelines, agency policies, and best practices for wound care.
  • Infection Control:
    • Implement appropriate infection control measures to prevent wound infections and promote a safe environment for patients.
    • Monitor for signs of infection, such as redness, increased drainage, or fever, and take necessary actions according to physician orders.
  • Continuous Learning & Professional Development:
    • Stay current with best practices in wound care and home health nursing by participating in ongoing education, workshops, and conferences.
    • Seek out training opportunities to enhance expertise in advanced wound care techniques and technologies.
Qualifications
  • Education & Certification:
    • Current RN licensure in CT.
    • Certification in wound care (e.g., WOCN, CWCN) required.
    • BLS (Basic Life Support) certification required.
  • Experience:
    • Minimum of 2 years of nursing experience in home health or wound care nursing.
    • Experience with chronic wound management, pressure ulcers, diabetic foot ulcers, surgical wounds, wound vacs, and other complex wound types.
  • Skills:
    • Strong clinical knowledge of wound care and treatment modalities.
    • Excellent communication and interpersonal skills for interacting with patients, families, and the healthcare team.
    • Ability to assess, plan, and deliver individualized patient care effectively.
    • Proficiency in electronic health record (EHR) systems and documentation.
Working Conditions
  • Home health setting with travel to patient homes.
  • Flexible work hours, which may include evenings or weekends depending on patient needs.
  • Potential exposure to infectious diseases, requiring adherence to infection control protocols.

Salary: $100,000- $110,000 per year
