Overview

Health Care Connectors is seeking a compassionate and skilled
Home Health Nurse Practitioner
to join our growing clinical team. The ideal candidate is clinically strong, organized, and passionate about delivering high-quality, patient-centered care in the home setting.

• Perform comprehensive patient assessments in the home, including physical exams, medication reviews, and chronic disease management.
• Act as the
  primary care provider
  for patients who do not currently have a PCP, ensuring continuity of care and ongoing clinical oversight.
• Complete and maintain
  OASIS documentation
  accurately and on time (required).
• Develop, implement, and evaluate individualized patient treatment plans.
• Collaborate with RNs, therapists, physicians, and other interdisciplinary team members to coordinate care and improve patient outcomes.
• Provide education to patients and families to support disease management and promote independence.
• Participate in in-office case conferences, training, and team meetings (approx. 25% of workload).
• Utilize
  Homecare Homebase (HCHB)for documentation and workflow (preferred).
• Ensure compliance with all federal, state, and agency standards and regulations.
• Provide on-call support if needed (optional—customize as needed).

• Active, unrestricted
  Nurse Practitioner license in Connecticut (CT).
• National certification (FNP, AGNP, or equivalent).
• Minimum1–2 years of clinical experience
  ; home health experience strongly preferred.
• OASIS experience required.
• Homecare Homebase (HCHB)experience preferred.
• Excellent clinical judgment, communication, and organizational skills.
• Valid driver’s license, reliable vehicle, and ability to travel throughout the Connecticut service area.
• Ability to work independently in a field-based environment.
