Job Summary:
We are seeking a compassionate and dedicated PD/HHD Nurse to join our dialysis team. The ideal candidate will possess strong clinical skills and a deep understanding of peritoneal dialysis (PD) and home hemodialysis (HHD) and procedures. The PD/HHD Nurse will be responsible for providing comprehensive care, education, and support to patients undergoing dialysis treatment in a home setting. This role requires a proactive approach to patient management, strong communication skills, teamwork, and a commitment to patient safety and education.
Key Responsibilities:
- Assess patients’ health status and monitor vital signs regularly.
- Provide education and training to patients and families on peritoneal dialysis and home hemodialysis techniques, equipment, and care plans.
- Develop individualized care plans and adjust as necessary based on patients’ needs and responses to treatment.
- Initiate, monitor, and discontinue dialysis treatments in accordance with established protocols.
- Ensure proper functioning of dialysis equipment and perform equipment maintenance as necessary.
- Document patient responses to treatment and any complications, reporting significant changes to the physician.
- Conduct patient education sessions to ensure understanding of the dialysis process, dietary restrictions, medication management, and infection prevention.
- Educate patients on recognizing and responding to symptoms of complications, such as infections or fluid overload.
- Work closely with physicians, social workers, dietitians, and other healthcare professionals to provide holistic care to patients.
- Participate in interdisciplinary team meetings to discuss patient progress and adjust care plans accordingly.
- Participate in quality improvement initiatives and adhere to safety protocols and standards.
- Maintain accurate documentation and records in compliance with regulatory requirements.
- Provide emotional support to patients and families, addressing concerns and providing resources for additional help.
- Advocate for patients and ensure they have access to necessary community resources.
Qualifications:
- Associate’s or Bachelor’s Degree in Nursing (BSN preferred).
- Current registered nurse (RN) licensure in Indiana.
- Minimum of [1-2] years of nursing experience, preferably in dialysis or a related field.
- Experience in home health nursing is a must.
- Certification in Nephrology Nursing (CNN) or related certification preferred.
- Basic Life Support (BLS) certification required;
Advanced Cardiac Life Support (ACLS) is a plus.
