Humana's Primary Care Organization is a fast-growing provider of senior-focused, value-based care, with 340+ centers across 15 states under the Center Well and Conviva brands. We're looking for a full-time Paneled Primary Care Nurse Practitioner who is passionate about caring for seniors, building long-term patient relationships, and managing a dedicated panel in a value-based primary care model. This onsite position is located at our Greer, South Carolina clinic and follows a Monday-Friday schedule.

You will report directly to the Associate Medical Director and work with an interdisciplinary care team to deliver high-quality, patient-centred care.

Manage Your Panel: Build meaningful, longitudinal relationships with your own panel of patients and directly help their health outcomes.

Team-Based Care: Collaborate with physicians, care coaches, behavioural health, and specialists to deliver whole-person care.

More Time with Patients: Lower panel size and visit volume support deeper engagement and proactive care.

• Manage primary care services to a designated panel of senior patients
• Improve panel performance through quality metrics (e.g., HEDIS, STARs, risk adjustment) and patient engagement plans
• Coordinate care across interdisciplinary teams, including specialists, behavioural health, and care management

• Graduate of an accredited Nurse Practitioner program (MSN or DNP)
• Current national certification (AANP or ANCC)
• Active, unrestricted NP license in South Carolina
• DEA license and Medicare/Medicaid credentials (or ability to obtain upon hire)

• 2+ years of Nurse Practitioner experience
• Experience managing a patient panel in a primary care
• Background in Medicare Advantage or geriatric-focused care
• Familiarity with HEDIS quality metrics, STAR ratings, and Medicare guidelines

The compensation range below reflects a good faith estimate of starting base pay for full time (40 hours per week) employment at the time of posting. The pay range may be higher or lower based on geographic location and individual pay will vary based on demonstrated job related skills, knowledge, experience, education, certifications, etc.

$136,200 - $187,400 per year

This job is eligible for a bonus incentive plan. This incentive opportunity is based upon company and/or individual performance.
